are you refer to this then i don't understand it what did you do? Did you pull the death lever in the grand hall? (looks like you did) (its the last lever -- right across from where the element of earth is) Its called death lever because of that reason. Pulling it either kills Vivi or locks him out....

haven't seen any hydras or worms
i used those 4 levers beside the hand there sent vivi high up to use another 4 levers
and used a ladder there come to a lever gives a flyby
